PLEASE NOTE: If you are registering a new Student Organization the follow documents must be completed and uploaded to the form below before it can be submitted:
Authorization to Release Limited Information
This form must be submitted to Campus Facilities (email@example.com) in order to get tables and chairs, as well as other items, if needed at an event or meeting. Campus Facilities requests that forms be submitted two weeks in advance. In order to submit a Facilities Request Form you must first submit a Room Request Form (above) and wait to receive your confirmation number. If you need to see a floor plan click here.
This form must be signed and submitted to the Office of Student Engagement (248-204-4115 or firstname.lastname@example.org) in order to request projectors, microphones, or other equipment need for an event or meeting. Media Services requires that forms be submitted at least 3 days prior to event date.
This form is to reserve "Blue," the Lawrence Tech Mascot, for an event. There is no fee to reserve "Blue," but the mascot must have an escort at all times. Donations for maintenance are appreciated.
This form is if to reserve Miss Lawrence Tech, for an event. There is no fee to reserve Miss Lawrence Tech, but donations are welcome. Donations will assist in funding the pageant scholarships.
This form is to reserve "The Lawrence Tech Dance Team" (LTDT). There is no fee to reserve LTDT, however donations are used toward costume maintenance, and team skills building.
Active organization have the opportunity to host events funded by Student Government. All submissions are due by 12PM the Friday before the next Student Government meeting. Please follow the Money Request Guidelines and contact email@example.com if you have any questions. Request funds here.
If your organization has a Funds Request that has been approved by the Senate, an organization member must complete and submit a Money Reimbursement Form to be reimbursed.
