In the Template section in DocuSign (after logging in with your LTU credentials), there will be an Enrollment Services Folder which contains the following documents:

Certificate, Concentration, and Minor Authorization Form

This form is used to Add a new, Revise a current, Inactivate a current, or Reactivate a former certificate, concentration, or minor.

Course Catalog Authorization Form

This form is used to Add a new, Deactivate a current, or revise any aspect of a course including: Course Number, Course Title, Course Description, Prerequisites, Co-requisites, Restrictions, Technical Requirements, Course Fees, or Credit Hours.
