THERE WILL BE NO AV TECH SUPPORT WITHOUT SUBMISSION OF AN OFFICIAL MEDIA SERVICES EQUIPMENT REQUEST FORM (MSER).
Forms should be emailed to the Media Services Dept. composite account: firstname.lastname@example.org, and NOT to individual member accounts. MSER forms can be faxed (248) 204-3026 or dropped off in office T124.
A minimum of one week's notice (5 working days) for projection-only requests is required. All other requests requiring the sound system, microphones, theatre lighting, or any other technology require two week's notice. Other requests involving outside talent, stage productions, etc. may require more advanced notice. Equipment and services are FCFS so please plan accordingly. Please submit a separate MSER form for each date and location needed.
All facilities rentals must be requested through Special Events and Services phone (248) 204-2311.

Instructions on filling out Media Services Equipment Request Form Online:
1) Click the Media Services Equipment Request Form PDF link below.
2) When you click the PDF link, you will have the option to SAVE the PDF to your computer or OPEN With.
Choose open with ADOBE ACROBAT.
3) Fill out form and save it to your computer.
4) Go to your email account and attach and email your Media Services Equipment Request PDF to: email@example.com
