Summer Camp Registration Form Rules and Regulations                                                            Field Trip Consent Form Health and Medical Information Parental Consent and Release You can submit your registration form (and all other forms) and payment in one of three ways: 1. Mail the completed registration form (and check if not paying by credit card) to: Lisa R. Kujawa Lawrence Technological University 21000 West Ten Mile Road Southfield, MI 48075-1058 2. Scan the completed registration form and email it as an attachment to kujawa@ltu.edu. 3. Fax the completed registration form to Lisa R. Kujawa at 248.204.2018. Once your registration form is received, you will be sent a confirmation letter and more information.  ABSOLUTELY NO REFUNDS WILL BE GIVEN AFTER REGISTRATION IS COMPLETED.
