Forensic Summer Science Institute 2008
June 23 – July 1

INSTRUCTIONS
This application has three parts. ( format)

PART 1 is completed by the student.

PART 2 is completed by a science teacher (preferred) or counselor familiar with the student’s abilities. The student’s high school transcripts must be attached.

PART 3 is completed by the student’s parent or guardian once the student has been accepted to the program. Applicants who are admitted will be notified by telephone or email. Part 3 must be completed within seven days of acceptance and include the $250 nonrefundable registration fee. The registration fee can be paid with a check or money order.

Complete each part of the application electronically, print it out, and send it, with all required documents, to:
