APPLICATION
FORENSIC SUMMER SCIENCE INSTITUTE 2010

Two separate sessions, select one or the other:

1) June 28 - July 2

    OR

2) July 12 - July 16

INSTRUCTIONS
This application has three parts.

Part 1  is completed by the student.

Part 2  is completed by a science teacher (preferred) or counselor familiar with the student's abilities. The student's high school transcripts must be attached.

Part 3  is completed by the student's parent or guardian once the student has been accepted to the program. Applicants who are admitted will be notified by telephone or email. Part 3 must be completed within seven days of acceptance and include the $200 nonrefundable registration fee. The registration fee can be paid with a check or money order.

Complete each part of the application electronically, print it out, and send it, with all required documents, to:
