On-Campus Employment Instructions
This page is designed for LTU Faculty, Administrators, and Staff for student employment on campus.
U.S. Citizen Employees
Please review all employee paperwork to ensure correctly completed forms. Include all copies of authorized documents (most common: social security card & driver's license OR passport) required for the hiring process. Direct supervisors are responsible for completing the Request for Personnel Action Form (RPA). The employment packet and RPA are required to be sent together via interdepartmental mail to the Provost's Office ATTN: Debbie Pryzbylo.

Note: A blank "voided" check must be included with the direct deposit form if student opts for this method to receive payment.
International Student Employees
Please review all employee paperwork to ensure correctly completed forms.
Required Copies of Documents to include in employee application for employment: I-20/DS2019, Current Visa, I-94, & Social Security Card
International student employees are required to obtain a social secruity card from the Social Security Administration Office. Two letters of authorization are required to apply.
Authorization Form Example (replace all information in yellow)
Please send the completed signed Authorization Form (not the original) via email to the Office of International Programs (international@ltu.edu) for the second letter of authorization. The student employee will be notified by the Office of International Programs when to pick up both authorization letters.
Note: Authorization forms must be signed by the direct supervisor (no electronic signatures). Start date must be dated minimum 1 week in the future for processing purposes.
Social Security Cards typically take 2-4 weeks for the recipient to receive through the U.S. Postal Service. The student is not to begin work until the social security card has been received by the direct supervisor and all paperwork has been processed by the Human Resources Department.
The student is responsible for providing the signed social security card to the direct supervisor to be copied. Complete the remaining sections of the employee paperwork. The Request for Personnel Action Form above may now be completed. Forward ALL forms to the Provost's Office through interdepartmental mail, ATTN: Debbie Pryzbylo.
